Changes to the PPC application form

The prescription prepayment certificate (PPC) application form (FP95) has changed.

Pharmacy contractors can use the new form to collect the customer’s details before completing the application online.

Please do not send forms directly to the NHS Business Services Authority (NHSBSA), as we cannot issue a PPC using the form, and they will be returned for you to do the application online.
